How to Obtain Authorization for a CES Device

For USA Customers

Please note that a licensed healthcare provider must authorize your purchase before we can ship your device. You may provide the authorization to Caputron before or after your purchase. Online authorization is available for $18 through the following telemedicine referral website: CES Online Authorization

How It Works

Step 1. Complete and submit the online Authorization Form

Step 2. A healthcare provider will review your form, typically within 24 hours

Step 3. If approved, your CES device will be shipped

For International Customers

No device authorization is needed. Simply add your favorite CES device to your cart and checkout.
